I would say "yes" to all three.
At the risk of sounding like a broken record lately, the overall fit and finish quality would suggest that this is a product of the Matsumoku plant in Japan. There are certain elements of this guitar which support that hunch; those solid brass knurled knobs and brass nut in particular. These were prominent features on several Mats' production lines including Aria Pro II, Westone, and Vantage in the late 70's and early 80's.
Mini-toggle switches for coil tapping, phase reversal, and active electronic switching were also a regular feature of the Mats designs of the day, something that was seen as radical ideas at the time. If you search these forums you'll find coil splitting functions on other LP copies. This worked pretty well on "Strat" style models but didn't make too much sense to LP players so it appears that it was dropped rather quickly.
